Huynh Dental Clinic takes the protection of the information entrusted to it seriously. This policy explains what this website collects, why, for how long, and which rights you can exercise.

This policy covers the website. Health information recorded in your dental file is governed by the dentist’s professional obligations and by the Code de déontologie des dentistes, and is subject to professional secrecy.

Under the Act respecting the protection of personal information in the private sector (CQLR, c. P-39.1), as amended by Law 25, a person within the clinic is responsible for the protection of personal information.

What the site collects

Simply browsing the site collects nothing about you. We use no audience measurement tool, no advertising pixel and no tracking cookie. No resource is loaded from a third-party server — no font, no map, no external script.

A single form collects information: the callback request form on the Contact page. It has the following fields:

  • your name (required);
  • your telephone number (required);
  • your email address (optional);
  • the time of day that suits you for a callback;
  • the reason for your request, in free text (optional).

Do not enter any health information in this form. The “reason” field is for indicating the kind of appointment you want — an examination or a cleaning, for instance — not for describing a medical condition. Ordinary email does not offer the safeguards that kind of information requires.

Why we collect it

Solely to contact you and arrange an appointment. This information serves no other purpose. We do not send newsletters, do not carry out marketing, do not sell or disclose your information to third parties, and do not use it for any advertising purpose.

Consent

The form requires an explicit checkbox before it can be sent. You may withdraw your consent at any time by writing to the person named above; we will then destroy the information collected through the form, subject to the following.

Retention and destruction

Callback requests are sent by email to the clinic. They are not stored on the website’s server. The email is kept for as long as is needed to follow up on your request, then destroyed.

If you become a patient of the clinic, your contact details are then added to your dental file, the retention of which follows the rules applicable to dentists’ professional records rather than this policy.

Cookies

The site sets no advertising or analytics cookie. One technical cookie is used: a session cookie, set at the moment you submit the form, whose sole purpose is to display the confirmation or error message on the following page. It expires when you close your browser.

If you never submit the form, no cookie is set. That is why this site carries no cookie consent banner: it does not need one.

Preventing automated submissions

To limit bulk submissions by robots, the site temporarily keeps a salted cryptographic fingerprint of the IP address that submitted the form, for no more than one hour. The address itself is neither stored nor recoverable from that fingerprint, which is deleted automatically afterwards.

Security and hosting

The site is served over HTTPS: what you send is encrypted in transit. It is hosted by SiteGround, whose servers process the data submitted through the form. The clinic ensures the hosting arrangement carries appropriate protective measures.

Your rights

At any time and free of charge, you may:

  • be told what information we hold about you, and access it;
  • ask for it to be corrected if it is inaccurate, incomplete or ambiguous;
  • withdraw your consent to its use;
  • ask for it to be deleted where keeping it is no longer justified.

Address your request to the person named above. We will respond within 30 days. If our answer does not satisfy you, you may file a complaint with the Commission d’accès à l’information du Québec.

Confidentiality incidents

In the event of an incident presenting a risk of serious injury, the clinic will notify the people concerned and the Commission d’accès à l’information, and will record the incident in the register required by law.

Any change to this policy is published on this page, and the date shown at the top is updated accordingly.
